CLINIC REIMBURSEMENT PROCESS - PLEASE READ IN FULL!

Please ensure you communicate directly with the Coach Coordinator (coach@coqmha.org), for Coach Clinic reimbursements (Not paid by team fees).

*Coaches must be rostered to a team in order to qualify for clinic certification and reimbursements. Each team will be provided a maximum reimbursement for 1 Head Coach & 2 Assistants*
- Coaches who are receiving honorariums are NOT eligible for reimbursement.
Coaching clinic reimbursements will be paid directly to the coach at the end of the season (after March 31), via e-payment, provided the request has been approved by the Coach Coordinator.

-Coaches must submit this Jotform.
-Coaches must have completed the E-payment Set-Up Form (The Coach Coordinator will provide this to you)

-If a coach renewed Respect in Sport in the current season, they must submit that within this Jotform - NOT - the form for other team officials, UNLESS they did not take any coaching certs that season. If they did not take any coaches certs in the current season, submit your RISAL to your team for reimbursement.

Deadline to submit the reimbursement request is March 20th - NO EXCEPTIONS. Late submissions will not be processed!

NOTE:

You must submit coach certs to proceed with this form. If you only have RISAL submit those to your team. DO NOT submit them to the Coach Coordinator.

PROOF OF PAYMENT IS REQUIRED. PLEASE UPLOAD YOUR RECEIPTS.

Only volunteer coaches will be reimbursed. If you are coach receiving an honorarium, do not submit this form.

Coaching Certs eligible for reimbursement are:
- Dev 1
- Coach 1
- Coach 2
- Checking
Do not submit HP 1 certs; CMHA does not cover that clinic.

Do not submit for clinics that were not required for your role (for example, Assistant coach taking Checking clinic, or U9 coach taking Coach 2). CMHA only covers the clinics that are required for each rostered role.

Do not submit if you are a "paid" coach.
