Please fill it out to the best of your ability, if you are unsure of what to put for a section please just answer 'unsure' and we can discuss further during a consultation. Submitting this form does not ensure you an appointment, this is simply a request and I will reach out to continue the booking process. A deposit will be required for new clients in order to complete the booking process, a desposit form will be sent over once dates have been discussed.
