2025 Expression of Interest
Please see below the WOSHC expression of interest form for bookings in 2025.

As part of your enrolment at our service, we require you to confirm acceptance of the following items each year, in order to be eligible to receive Government funding (if applicable). Acceptance of these items as well as some of the other information in the enrolment form can be used as a Complying Written Arrangement.
Enrolment Form Review
Please review your child/children’s ENTIRE enrolment form via your QK Enrol (My Family Lounge) account and confirm via the checkboxes below your acceptance of each of these items. This will indicate that you have checked and confirmed your child/children’s ENTIRE enrolment form.

Please access your account via a desktop browser as the enrolment form does not show in the App.

- A completed Expression of Interest form has been submitted

- The service has been provided with current and up-to-date Medical Management plans, Anaphylaxis Medical Administration forms and Risk Minimisation plans, and/or Court or Parenting Orders.

- Child/Children's enrolment form has been reviewed and all fields marked with a red asterisk '*' has been answered and updated if required.

Please allow 10 business days for the processing of this form.

You will receive an email to either request further information or to confirm your bookings.
