I, {parentguardianName}, acknowledge that in consideration of being allowed to participate in activities organized by APEX SOCCER ACADEMY INC ("Activity" or "Activities"), which include competitive and recreational sports, physical exercise, and training, I agree to the following terms:

1. I understand the nature of the Activities and affirm that I am in good health and proper physical condition to participate. I agree that if I ever believe conditions are unsafe, I will cease participation immediately.

2. I fully comprehend that:
- The Activity carries risks of serious bodily injury, including but not limited to permanent disability, paralysis, and death ("Risks").
- These Risks may arise from my actions or inactions, the actions or inactions of others, the conditions of the Activity location, or the negligence of the "RELEASEES" named below.
- There may be other risks and economic losses not presently known to me or not readily foreseeable.
- I accept and assume all such Risks and responsibility for any losses, costs, or damages incurred by myself or by the Minor in connection with the Activity.

3. I hereby release, discharge, and promise not to sue APEX SOCCER ACADEMY INC, its administrators, directors, agents, officers, members, volunteers, employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity occurs (collectively referred to as "Releasees"), from all liability, claims, demands, losses, or damages on my account, whether caused or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise. I further agree to indemnify, defend, and hold harmless each Releasee from any litigation expenses, attorney fees, loss, liability, damage, or cost they may incur due to any claim made by me or on my behalf.

4. I grant to APEX SOCCER ACADEMY INC and its successors and assigns the irrevocable, perpetual, royalty-free, worldwide, sublicensable, and unrestricted right to use and publish the Minor's or my name, voice, likeness, image, photographs, or any materials in which the Minor or I may appear, for editorial, trade, advertising, or any other purpose and in any manner and medium, including alterations and composites, without needing my inspection or approval. I release APEX SOCCER ACADEMY INC from all claims and liability relating to the foregoing uses, including any claims based on rights of privacy or publicity.

I have carefully read this Agreement, fully understand its terms, and acknowledge that by signing it, I am relinquishing significant legal rights. I sign this Agreement freely and without any assurance or inducement, intending it to be a complete and unconditional release of all liability to the fullest extent permitted by law. I agree that if any part of this Agreement is deemed invalid, the remainder shall continue in full force and effect.
MINOR RELEASE

As the Parent and/or Legal Guardian of the Minor, I acknowledge the nature of the Activities and the Minor’s capabilities, believing them to be qualified, in good health, and in suitable physical condition to participate. I hereby release, discharge, covenant not to sue, and agree to indemnify, save, and hold harmless each of the Releasees from all liability, claims, demands, losses, or damages on the Minor’s behalf, whether caused by the negligence of the Releasees or otherwise, including negligent rescue operations. Furthermore, I agree that if any claim is made against the Releasees by myself, the Minor, or anyone acting on the Minor’s behalf, I will indemnify and hold harmless each Releasee from any litigation expenses, attorney fees, liability, damage, or costs incurred as a result of such claim.
