    For AFL South East Umpires ONLY

  • Please use this form if you are an AFLSE league appointed umpire looking to refer an incident. This must be done by 12PM the first business day after the incident.

     

    Referrals can only be made for games with official match footage. 

     

    This form can ONLY be filled out by league appointed umpires (regardless of discipline - field, boundary or goal). Clubs looking to refer an incident must follow the process as stipulated in the MPFNL Football Operations Manual (page 50, 64. Match Review Officer)

     

    Under NO circumstances are umpires to refer an incident at the request of a club.
