Risk Acknowledgement and Consent to Participate

I/We will realize that there are numerous risks involved in participating in the selected sport. These risks could involve(but are not limited to): sprains, contusions, broken bones, lacerations, concussions, permanent disability, internal injuries, paralysis, and possibly death. These risks could impair my/our child's future abilities to earn a living, engage in business, social and recreational activities and to generally enjoy life. I/we have been informed about the various risks associatied with our child's participation in the above listed sports and the potential injuries that may occur.

I/We will assume all responsibility and certify my/our child is in good physical condition and has undergone a sports physical in the past two years. Further, I/we are unaware of any medical condition that would inhibit my/our child's participation.

As a consition of our child's voluntary participation in the selected sport, I/we agree to accept all the previosuly mentioned risks as a condition of my/our child's particpation.

Heads Up Concussion Information

Parent Agreement

I/We have read the Concussion Fact Sheets for Parents and understand waht a concession is and how ti may be caused. I also understand the common signs, symptoms and behaviors.

I/We agree to seek medical treatment if a suspected concussion is reported to me.

I understand that my child cannot return to practice/play until providing written clearance frm an appropriate health care provider to his/her coach.

I understand the possible consequaences of my child returning to practice/play too soon.

Athlete Agreement

I have read the Concussion Fact Sheet for Athletes and understand the mportance of reporting a suspected concussion to my coaches and my parents/guardian.

I understand that I must be removed from practice/play if a concession is suspected. I undertand that I must provide written clearance from an appropriate health care provider to my coaach before returning to a pratice/play.

Spartan Athletic Uniform Agreement
Your student has joined a Spartan Sports team and will be issued an Athletic Uniform. Each item will be returned to their caoch at the end of the sport season they are in. Please sign below aggreeing to returning any uniform pieces and aknowledging the fee associated with unreturned or damaged uniform pieces.
I (We) agree to pay a fee of $30 for each article of my (our) child's uniform that is not returned by the end of the sports season. (cheerleading fees are handled separately)*

Physical Examination Form

ALL ATHLETES are required to have a Physical Examination Form on file.

Please click on link and print form to be given to your physician. Hard copies of this form were included in the summer packet and or can be downloaded here and on the school website.

The Physical Examination Form is required to be on file at school before the start of the sports season and before participating in the first practice.

Forms should be submitted to the main office at school before first team practice.
