Waiver to Volunteer

I, {name}, desire to volunteer for Northwestern and engage in the activities related to being a volunteer. (Current employees of Northwestern wishing to volunteer should refer to policy 519 in the employee handbook and should not fill out this form.) I hereby voluntarily execute this Volunteer Waiver under the following terms:

1. Statement of Faith & Representation: I confirm that I have read and agree to Northwestern’s Doctrinal Statement and Declaration of Christian Community. I understand that, as a volunteer, I represent Northwestern and am expected to reflect its values.

2. General Release of Liability: I release and hold harmless Northwestern, along with its successors and assigns, from any and all liability, claims, and demands of any kind, whether in law or equity, arising from my volunteer service.

3. Waiver of Personal Injury Claims: I understand that this waiver discharges Northwestern from any liability or claims related to bodily injury, personal injury, illness, death, or property damage that may occur during my volunteer activities.

4. No Obligation for Financial or Medical Support: I acknowledge that Northwestern does not assume responsibility for providing financial, medical, health, or disability assistance in the event of injury, illness, death, or property damage. I also release Northwestern from any claims related to first aid or emergency medical services provided during my volunteer service.

5. Assumption of Risk: I understand that my volunteer activities may involve risks. I voluntarily assume all risk of injury or harm and release Northwestern from all liability resulting from these activities.

6. Media Release & Use of Likeness: I grant Northwestern full rights to any photographs, video, or audio recordings taken of me during my volunteer service, including the use of such materials for any purpose without compensation.

7. Governing Law: I agree that this waiver is intended to be as broad and inclusive as permitted under Minnesota law and will be governed and interpreted according to the laws of the State of Minnesota.

8. Confidentiality Agreement: I agree to maintain the confidentiality of all information encountered during my volunteer service. I will not disclose any personal, financial, or sensitive information to unauthorized individuals. I understand that a breach may result in termination of my volunteer role.

9. Collaborative Conduct: I will foster unity and collaboration by working respectfully with staff and volunteers from diverse denominations and backgrounds.

10. Respect for Leadership: I will demonstrate professionalism and respect by cooperating with staff and volunteer leaders in supervisory roles.

11. Severability: I agree that if any provision of this waiver is found to be invalid by a court, the remaining provisions will remain in full force and effect.

12. Background Check Requirement: I understand that volunteering with Northwestern is contingent upon successful completion of a background check, which will be conducted by HireRight.

13. Right to Terminate Volunteer Service: I understand that Northwestern may terminate my volunteer service at any time, with or without notice, for any reason, including but not limited to policy violations, confidentiality breaches, misconduct, or safety concerns.

If the volunteer is a minor (age 14 or above), a parent or legal guardian consent signature & information are required below on behalf of the minor.

Parental Consent and Release for Minor Volunteer Participation

I am the parent or legal guardian of the minor named below and give my consent for my child to participate in volunteer activities with Northwestern. I understand that I am required to be present with my child at all times and that I am fully responsible for my child’s behavior and actions during the volunteer experience.

I acknowledge that participation in volunteer activities may involve some risk. To the fullest extent permitted by law, I release and hold harmless Northwestern, its employees, and volunteers from any claims, demands, or liability arising from my child’s participation, including injury or illness, except in cases of gross negligence.
