Travel Planning Questionnaire

Thank you for your interest in planning your next vacation with Destination by Design. I love helping people plan for their vacations and put a tremendous amount of time and effort into planning the right vacation for you and those traveling with you.

To do this effectively, I need YOUR help! This questionnaire is designed to help you be as specific as possible with your information about the type of travel you're interested in, when you want to travel, and where you want to travel. The more specific you are about your travel wants, the better I can plan for your trip. 

Once your form is complete, I will get back to you with 2-3 options that best fit your needs and desires. Keep in mind rates vary and fluctuate based on supply and demand. Your rate will not be locked in until the deposit or full payment is processed.

Travel Insurance is not required, but it is highly recommended. I will include the price with and without insurance. If you choose to decline travel insurance, I will send you a waiver that indicates you are declining insurance. 

The trip planning services are FREE. I are compensated by the cruise lines, tour operators, resorts, and hotels that you select for your trip.

I do reserve the right to charge a service fee for my time involved with planning your trip under certain circumstances. The primary circumstance in which I would charge a travel planning fee is if you completely change the itinerary of your travel. (i.e. After completing this form you ask me to plan an all-inclusive Caribbean trip, but during the research phase you tell me that you want a vacation to Paris instead.) This type of change would require a Travel Planning Fee.

The trip will be charged by the vendor (i.e. Marriott, Carnival, Southwest, etc). Please allow 3-5 business days for your quote. *Note: I have an apples to apples best travel price guarantee; if you find a more cost effective price, please let me know.

 

Travel Planning Fee Schedule

US Domestic Travel:  $25

International Travel: $50

Group Travel (6 or more): $75

The fee will be charged before I begin researching the alternate travel destination and is non-refundable.
