1. In addition to filling out this form, you must upload at least one, and up to three, letter(s) of recommendation from another person who supports your nomination. PDFs and Word documents are accepted.
2. It is helpful to upload the nominee's resume as well. PDFs and Word documents are accepted.
ADDITIONAL ACTION REQUIRED
Once you submit this form, you will receive an email confirming all the information you submitted. Your nominee will receive an email at the address you provide for them asking them to complete a form signaling their commitment to the following:
- Participate on all program dates.
- Do a small amount of background reading.
- Participate in a class project that will require about 2 hours time a month outside of class time.
This commitment form is required for the nominee to be considered, so it is essential that you use the nominee's correct email address. You must also ensure the nominee follows through and completes the form.
