風險承擔與肖像授權同意書

本人已詳細閱讀且了解相關規定及風險，並同意遵守以下條款
I confirm that I have carefully read and understood the relevant rules and risks and agree to comply with the following terms.

風險知悉 
Risk Acknowledgment

• 臺灣巴克動物懷善救援協會重視動物、志工、參與者及工作人員之安全，並將採取合理的安全預防措施；惟本人了解，參與動物照護、志願服務及相關活動仍可能存在無法完全排除之風險。
  PACK Sanctuary Taiwan places the safety of animals, volunteers, participants, and staff as a top priority and will take reasonable safety precautions. However, I understand that participation in animal care, volunteer service, and related activities may involve risks that cannot be completely eliminated.

• 本人知悉在動物收容園區、公共區域、活動場地或辦公空間內，可能因地面不平、濕滑、使用工具或相關設施，以及其他突發情況而受傷；與動物靠近或接觸時，亦可能發生咬傷、抓傷、碰撞或跌倒等情形。本人將依自身健康及身體狀況審慎評估是否參與相關工作，如有不適，應立即停止並告知工作人員。
  I understand that injuries may occur at animal shelters, public areas, event venues, or offices due to uneven or slippery surfaces, the use of tools or facilities, or other unforeseen circumstances. Approaching or interacting with animals may also involve risks including, but not limited to, bites, scratches, collisions, or falls. I will carefully assess my health and physical condition before participating in any activity. If I feel unwell, I will stop immediately and notify a staff member.
  
  

安全規定及責任承擔
Safety Rules and Assumption of Responsibility

• 本人同意遵守協會之安全規定及工作人員指示，不擅自進入限制區域、接觸未經安排之動物，或操作未經指導之工具及設施，並採取合理預防措施，以維護本人、他人及動物之安全。
  I agree to comply with all safety rules established by PACK Sanctuary Taiwan and follow the instructions of its staff. I will not enter restricted areas, interact with animals without authorization, or operate tools or facilities without proper instruction. I will take reasonable precautions to protect myself, other people, and animals.
  
  
• 如因本人未遵守安全規定、工作人員指示，或因本人自身健康狀況及其他非可歸責於協會之事由，造成本人或第三人之人身傷害或財產損失，本人願依法承擔相應責任。若發生受傷、身體不適或其他事故，本人同意立即通知工作人員，並配合必要之醫療或緊急處理。本條不包含依法不得預先免除之責任。
  If my failure to follow safety rules or staff instructions, my health or physical condition, or any other circumstance not attributable to PACK Sanctuary Taiwan causes personal injury or property damage to myself or a third party, I agree to assume the corresponding responsibility in accordance with applicable law. If any injury, illness, or other incident occurs, I will immediately notify a staff member and cooperate with any necessary medical or emergency response. Nothing in this provision excludes or limits any liability that cannot legally be waived in advance.
  
  
  
  

肖像授權與同意
Image and Media Release

• 本人或未成年人之法定代理人同意協會於活動及志願服務過程中，拍攝、錄製及使用包含本人肖像、聲音、姓名或訪談內容之素材，並得進行必要之剪輯、修飾、重製、公開展示及公開傳輸，用於動物福利倡議、浪浪認養、志工招募、公益募款、活動紀錄及協會相關宣傳。
  I, or the legal guardian of a minor participant, authorize PACK Sanctuary Taiwan to photograph, film, record, and use materials containing my image, voice, name, or interview statements during activities and volunteer service. PACK Sanctuary Taiwan may edit, retouch, reproduce, publicly display, and publicly transmit such materials for animal welfare advocacy, dog adoption promotion, volunteer recruitment, fundraising, activity documentation, and other communications related to PACK Sanctuary Taiwan.

• 上述素材得使用於協會或合作單位之網站、社群媒體、電子報、新聞媒體、廣告、簡報、海報、刊物及活動現場，授權地區不限，自簽署日起生效，並為無償授權。本人得通知協會停止未來新增使用，但通知前已製作或發布之內容不受影響。相關攝影或影音著作之著作權歸實際著作權人所有，協會不得以歪曲、誤導或貶損本人名譽之方式使用。
  These materials may be used on websites, social media, email newsletters, news media, advertisements, presentations, posters, publications, at event venues, and through other channels operated by PACK Sanctuary Taiwan or its partners. This authorization is granted worldwide, takes effect on the date of signing, and is provided without compensation. I may request that PACK Sanctuary Taiwan discontinue any future use of the materials; however, such a request will not affect materials already produced or published before the request was received. Copyright in the photographs, recordings, and audiovisual works remains with the applicable copyright owner. PACK Sanctuary Taiwan shall not use the materials in aing a distorted, misleading, or defamatory manner.

未滿 18 歲之參與者，應由家長或法定代理人閱讀本同意書並簽署。請確認所填資料完整且正確，以保障相關權益。如對本同意書內容有任何疑義，雙方應依相關法令及誠信原則協議處理。

Participants under the age of 18 must have this form reviewed and signed by a parent or legal guardian. Please ensure that all information provided is complete and accurate to protect the rights and interests of all parties. Any questions or disputes regarding this form shall be resolved through mutual consultation in accordance with applicable laws and the principle of good faith.
