Thank you for your interest in our Dogs’ Day Out Program. This program provides vital enrichment for dogs in our care and helps us learn more about them to get more information to help them get adopted.
By participating in this program, I agree to the following:
- I will carefully read all documents and carefully follow all instructions that the YCSPCA provides to me.
- I will have the dog under my supervision at ALL TIMES.
- I will never leave children unattended with the dog.
- I will not take the dog to a dog park or any other off-leash area or let the dog off leash unless confined in my home or in my securely fenced and inspected yard.
- I will not leave the dog unattended in a fenced yard.
- I will not allow nose-to-nose contact with any other animal, dog introductions, or meet-and-greets while on the Dogs’ Day Out. I will tell the interested person(s) to contact the YCSPCA for more information and to schedule a meet-and-greet.
- I will pick up and return the dog at the agreed time.
Pick-up is at 9:00 a.m., and drop-off is no later than 4:00 p.m.
- I will transport the dog in a safe manner by either crating the dog or safely restraining the dog in the back seat. I will maintain at least the state-required minimum amounts of automobile insurance and will not hold YCSPCA responsible for any damage to my vehicle by the dog.
- I will not hand off the dog to a potential adopter. If someone is interested in adopting the dog, I will give them information on our adoption process and a business card/information sheet.
All adoptions must go through a client services counselor at the YCSPCA.
- I will actively participate in marketing the dog. I give the YCSPCA permission to post/share any pictures or videos I take while on the Dog’s Day Out solely for marketing and promotional purposes.
I will present the dog positively to potential adopters and members of the public.
- I confirm that I am willingly offering to take a dog on a Dogs' Day Out and that I do not expect any goods or services in return.
- I acknowledge that the decision regarding who adopts the dog rests with the YCSPCA.
- I will immediately report any bite that occurs to myself, another person, or an animal to a YCSPCA manager on duty.
If the dog gets loose or is lost, I will immediately notify the foster and rescue coordinator or the manager on duty.
- I will use reasonable care to ensure the safety and well-being of the dog in my care, the safety of other dogs, and the safety of the people the dog meets.
- I understand that our YCSPCA medical team provides all medical care for our dogs, and the YCSPCA will NOT reimburse me for the cost of ANY outside medical care.
- I will return the dog to YCSPCA immediately if the dog exhibits any signs of an emergency, as outlined in our orientation. Any non-emergency medical or behavioral concerns will be reported to the foster and rescue coordinator or the manager on duty when the dog is returned.
- I will only utilize positive reinforcement training techniques. I will never verbally or physically punish or harm the dog in my care.
- I will have the cell phone, whose number is listed below, on my person at all times during the Dogs’ Day Out. If my number should change, I will immediately notify the foster and rescue coordinator to update my records. I am giving YCSPCA permission to contact me, if necessary, while on the Dogs’ Day Out.
- I am at least 18 years of age.
By agreeing to short-term foster in partnership with the YCSPCA, I agree to the following:
- I agree to be fully responsible for the safety and well-being of the dog I take on a Dogs’ Day Out.
- I will provide a safe, loving, humane environment with adequate water, rest, shade, and shelter at all times.
- I will keep a safe distance between the dog in my care and any other public dog.
- I will adhere to all state and local animal laws.
- I will promptly notify the YCSPCA of any signs of illness, behavioral issues, or concerns if the dog is lost and/or if the dog bites anyone.
Transfer of Animals
Animals cannot be transferred to the custody of another person, shelter or other entity without prior consent and permission from the YCSPCA. All animals are the property of YCSPCA.
Return of Animals
All dogs in the YCSPCA Dogs’ Day Out program must be returned by 4:00 p.m. on the day of the Dogs’ Day Out.
I agree that I am temporarily fostering this pet for the YCSPCA and do not have any right of ownership over the animal.
I further agree that the YCSPCA rights in and to my foster pet are superior to mine.
Personal or Property Damage and/or Injury
I agree that accidental animal bites or other injuries to humans and other animals do occur, and I agree to hold harmless, indemnify, and protect the YCSPCA from any claim or suit filed by someone as a result of such incident.
In addition, the YCSPCA will not be responsible if the dog should damage or destroy property belonging to the volunteer caregiver/Dogs’ Day Out participant, their household, or others or if the animal shall transfer any disease, internal or external parasites to other animals and people in Volunteer Caregiver’s household.
I understand that if I am approved for short term fostering/Dogs’ Day Out program, this declaration represents a legal contract between me and the YCSPCA. I understand I am the designated caregiver for all animals owned and cared for by the YCSPCA.
I understand that if I am approved to short-term foster an animal, I must abide by this agreement, which applies to all animals I short-term foster with the YCSPCA. Termination of this contract is at the sole discretion of the YCSPCA.
Accuracy of Information
I acknowledge that the information provided on this application is correct to the best of my knowledge. If at any time the information I have provided changes, I will provide updated information to the YCSPCA foster and rescue coordinator.
By signing below, I acknowledge and represent that I have read and understood the foregoing release, sign it voluntarily, and agree to the indemnity and waiver of liability above. I also agree to all rules and regulations stated above:
