• Tattoo Consultation Form for Ella Rose

    PLEASE READ ALL OF THE FOLLOWING POLICIES BEFORE MOVING FORWARD WITH SUBMITTING THE FORM.

  • This form is to collect all your thoughts, ideas, and pictures; Along with properly displaying my policies and gulidelines for new tattoos/new clients.

    This form holds no quarantee you are accepted as a client, I will reach out to you personally to discuss further!

     

  • - $100 NONREFUNDABLE deposit is required to schedule. Deposits are taken off the last session if multiple sessions are needed. Deposits can be made with cash or Venmo @ellarose_tattoo

    -$150 an hour. $350-half day $700-full day rates

    - Reschedules can occur with 48 hour notice without deposit being affected. If no call no show or late cancellation occurs an additional deposit will be required to reschedule. Deposit is only valid for 2 reschedules, after the third reschedule another deposit will be required.

    - All payments can be made through Cash, Card, or Venmo. There is a 4% processing fee added to card payments.

    In order to help me streamline the booking process please fill out any relevant information I may need for your appointment below. Once this has been filled out you will be automatically put onto a waitlist and from then you will be contacted for scheduling the tattoo process.
