General Release
The undersigned agrees that Connect and Christ Covenant Church or any subdivisions thereof, shall be held harmless from any liability for damages to person or property to the participant named herein that might arise out of, in route to, in route from, while in residence, or as a result of, any involvement or participation by said participant in a program or activity of Christ Covenant Church, or any subdivisions thereof.
This authorization, consent, and waiver of liability will remain effective, unless revoked in writing by the undersigned, and delivered to the aforesaid agent.
In the event you can know longer attend; unfortunately, no refunds will be given.
If you would prefer to sign this waiver on paper, instead of online, you may request the paper form in writing. Once signed, please give the completed form to your campus director.
