  • I hereby indemnify, release and forever hold harmless Minds in Motion, LLC and and its owners, employees, representatives, and agents, from any and all claims of liability arising from any accident, personal injury, death, property loss or damage sustained by my child(ren)/myself/the minor child(ren) for whom I am a legal guardian (the “Participant”), while the Participant is engaged in activities connected with Minds in Motion, LLC; including classes, rehearsals, performances, or other activities, or is located on any of the premises utilized by Minds in Motion, LLC before, during, or after such activities.


    I understand that dance and movement activities have inherent risks of injury, and, being fully aware of all risk, I consent to have the Participant participate in the classes and activities offered by Minds in Motion, LLC; and I accept full responsibility for providing adequate health and accident insurance coverage for the protection of the Participant. By accepting and consenting to this statement, I declare that the Participant is in good health, with no physical conditions that might prevent participation in strenuous and rigorous movement activities and other training and performance connected with movement classes. I understand that I should be aware of the Participant’s physical limitations and agree not to exceed them.


    Further, I understand and acknowledge that because of the physical nature of dance and movement instruction; there may be physical contact between instructors and students during rehearsals, shows, workshops, productions, and especially during instruction. I understand that at times for proper instruction and safety, physical contact is required and necessary.


    I, and the Participant, accept the personal responsibility related to choosing appropriate protective health measures from communicable infections, as well as, the associated risks, known or unknown, of those protective health measures.  I agree to accept all related risk of communicable infections, as well as all liability should you or your child be exposed to, or infected with, a communicable infection, and you also release Minds in Motion, LLC from any such liability. 


    Furthermore, I and the Participant agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Minds in Motion, LLC.


    I have carefully read the contents of this waiver form and fully understand its contents. I understand that this is an assumption of risk and release of liability, and I accept it of my own free will.

  • Minds in Motion's Policies

  • Appearance:

    Appearance requirements promote safety, freedom of movement, and minimize distractions.  Dancers that do not arrive for class ready will be given a reminder and if they aren't able to follow the guidelines, then they will have to sit and observe class.

    -Dancers should bring a water bottle and arrive dressed for class whenever possible.  

    -All long hair should be secured away from the face - this includes bangs or layers that cover some or part of the eyes. 

    -Jewelry (with the exception of stud earrings) and watches should be removed before class. 

    -Labeling water bottles and dance shoes with the student's name is strongly recommended.   

    Wearing tights without dance shoes is not permitted, due to the increased risk of slipping they will NOT be permitted to participate in class.

    Attire and Shoe Requirements:

    • Jr Acro/Hip-Hop: Students may wear a leotard (any color) and tights or a fitted t-shirt (no cropped shirts, please) along with leggings/bike shorts and barefeet.  Dancers must wear fitted clothing for their safety - those wearing dresses or baggy clothing will NOT be permitted to participate in class. 
    • Jr Ballet/Jazz: Females should wear a leotard and may include a dance skirt.  Tights and pink ballet shoes are optional.  Males should wear a white t-shirt and black leggings or cuffed sweatpants.  Black ballet shoes are optional.  
    • Jr Ballet/Tap: Females should wear a leotard and may include a dance skirt.  Tights and pink ballet shoes are optional.  Males should wear a white t-shirt and black leggings or cuffed sweatpants.  Black ballet shoes are optional.  All dancers will be required to purchase (through the studio or independently) or borrow black tap shoes.  Wearing tights or socks with tap shoes is recommended.  If you're purchasing shoes, consider pull-on or velcro closure style shoes with a minimal heel.
    • Acro/Jazz and Jr Acro/Jazz students: Students may wear a leotard (any color) or a fitted t-shirt (no cropped shirts, please) along with leggings/bike shorts.  If your acro student is wearing tights, convertible or footless tights are preferred.  Barefeet are required.  Dancers must wear fitted clothing for their safety - those wearing dresses or baggy clothing will NOT be permitted to participate in class.  For the jazz portion of the class, half-soles are the preferred shoe of choice and may be purchased through the studio or independently.
    • Ballet and Contemporary/Ballet students: Females should wear a solid-colored leotard (short-sleeved is the most versatile), tights, and pink ballet shoes. Practice skirts and warm-ups may be worn during class as long as they aren't a distraction.  Hair should be secured in a bun, if possible.  Males should wear a white t-shirt and black leggings or cuffed sweatpants, and black ballet shoes.  If you are purchasing shoes, I recommend stretch canvas but other types are suitable.  Shoes with elastic ties will need to be tied, cut, and taped inside of the shoe - alternatively, choose a shoe without an elastic pull.
    • Choreo/Improv students: Students may wear a leotard (any color) and tights or a fitted t-shirt (no cropped shirts, please) along with leggings/bike shorts.  Barefeet, ballet shoes, or half soles are permitted.
    • Contemporary students: Students may wear a leotard (any color) and tights or a fitted t-shirt (no cropped shirts, please) along with leggings/bike shorts.  Mesh half sole or lyrical shoes are required.
    • Hip-hop students: Students may wear a leotard (any color) and tights or a  t-shirt (no cropped shirts, please) along with leggings/bike shorts/sweatpants.  Barefeet or studio dedicated shoes are permitted.  Hats are permitted in this class.  Outdoor shoes may NOT be worn on the studio floor.
    • Tap/Jazz students: Students may wear a leotard (any color) and tights or a fitted t-shirt (no cropped shirts, please) along with leggings/bike shorts. All dancers will be required to purchase (through the studio or independently) or borrow black tap shoes.  Wearing tights or socks with tap shoes is recommended.  If you're purchasing shoes, consider pull-on or velcro closure style shoes with a minimal heel. For the jazz portion of the class, dancers may be barefoot, wear ballet shoes, or black jazz shoes.
  • Studio Etiquette:

    -Please keep noise levels low while in the waiting area - enter and exit the studio quietly while classes are taking place. 

    -Treat the studio with respect - No gum, food, drink, or street shoes on studio floors. Place personal belongings out of the walkway. 

    -Treat the waiting area with respect - books and toys have been provided for your use while waiting and are not for use while eating.  Please return items to their designated spot after use and tidy up any messes.

    -Bullying, roughhousing, and any action harmful to others will not be tolerated. Students who are unable to follow directions will be given a warning and if the behavior reoccurs, they will be asked to observe class until they are ready to rejoin.  Reoccurring issues will be addressed with the student and their adult.

    -If your child has a fever or serious illness, please keep them at home. If they have an injury or condition that may affect participation, please notify the instructor. 

  • Payment:

    Please review the tuition info below:

    -Tuition includes four months of classes and the tuition rate is determined by the number of hours the dancer is enrolled in per week.
    -There will be four equal payments that are due on or before the first class in January, February, March, and April.  There is a discount if tuition is paid in full on or before the first class.
    -Late payments without communication with Laura will incur a $10 fee.
    -Accepted forms of payment are cash in a labeled envelope, check made payable to “Laura Althoff”, or Venmo @mindsinmotion (incurs a 4% fee).  Alternatively, there are financial aid and workshare opportunities.
    -Refunds will only be issued if the instructor cancels a class due to low enrollment.

     

  • Attendance and Home Practice: 

    The spring semester begins Monday, January 5th and will continue through Friday, April 24th.  There will be no classes the following dates: 3/16-3/20


    In the event of inclement weather, if Salida schools cancel in-person learning then studio classes will also be canceled and a make-up class will not be offered.  If there is a delayed start, classes will be held.  If after school activities are canceled, then dance classes will be canceled and a make-up class will not be offered.  The instructor will determine if classes are held if the inclement weather occurs on a non-school day.  If there are two or more inclement weather cancellations, then a makeup class will be scheduled.

    If the instructor needs to cancel a class due to illness or emergency, a make-up class will be offered or tuition will be adjusted at the instructor's discretion.  Class cancellations will be announced via email and on the Minds in Motion Facebook page.

    If you have a dancer enrolled in a Junior class (ages 3-5), an adult must be in the waiting area during the first two weeks of classes.  If your dancer is comfortable with you leaving during classes, try to be back for the last few minutes of class when the dancers show off their skills.  This helps to prepare them for performing in front of people and eventually the stage.

    Regular attendance helps the student to better develop their skills, instill discipline, and provide consistency - Aim to attend as many classes as possible. We all appreciate everyone's efforts to arrive on time and ready for class. If a student is late by 15 minutes or more, they may not participate in class due to the risks involved with missing warm-up/stretching. They may, however, stay and observe class.

    If the instructor needs to cancel a class due to illness or emergency, a make-up class will be offered or tuition will be adjusted at the instructor's discretion. Class cancellations will be announced via email and on the Minds in Motion Facebook page.

    If a student chooses to discontinue a class or classes, a refund will not be issued for any paid classes.  It is your responsibility to give the instructor notice of the change in enrollment.
