The Phoenix Award by Intermountain Sports Medicine
Behind exciting performances and glorious moments, athletes face numerous challenges that often go unnoticed. Some face devastating challenges that nearly break their spirit or threaten their athletic career. Athletes’ greatest achievements are not limited to their performance but also their feat to make an amazing comeback from such challenges. Like the Phoenix, the mythological bird that rises from ashes no matter how many times it burns up in flames, they prevail over unsurmountable challenges with extraordinary courage, resiliency, and willpower. The Phoenix Award was created to recognize and honor such athletes, who inspire us all through their incredible comeback stories. Please submit your nomination by sharing your comeback story that meets the following criteria, either in writing or a video format. By submitting your nomination, you give Intermountain Health your permission to share your story for the purpose of award selection and recognition.

The nominee experienced significant hardships such as injuries, surgeries, illnesses, or any other traumatic life events that negatively impacted their sport participation.

Describe ways in which they have grown through the hardship.
