Waiver and Release of Liability for Free Dancer Evaluation

Acknowledgment and Assumption of Risk:

I, the undersigned participant, hereby acknowledge and agree that participation in the free dancer evaluation conducted by Dance Beast involves inherent risks, including but not limited to physical injury, pain, discomfort, and potential exacerbation of existing medical conditions. I understand and voluntarily accept these risks.

Waiver and Release:

In consideration of being permitted to participate in the free dancer evaluation, I, for myself, my heirs, personal representatives, and assigns, hereby release, waive, discharge, and covenant not to sue Dance Beast, its owners, directors, officers, employees, agents, volunteers, and all other participants (collectively referred to as the "Released Parties") from any and all claims, liabilities, demands, actions, or causes of action that I may have, whether known or unknown, arising out of or related to any loss, damage, injury, or death that may be sustained by me as a result of my participation in the free dancer evaluation.

Medical Treatment:

I understand that Dance Beast does not provide medical or accident insurance for participants. In the event of an emergency, I authorize the Released Parties to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for my immediate care and agree that I will be responsible for payment of any and all medical services rendered.

Indemnification:

I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, liabilities, costs, expenses, or damages arising out of or in connection with my participation in the free dancer evaluation.

Photo/Video Release:

I grant Dance Beast permission to use photographs and/or video recordings of me taken during the evaluation for promotional purposes without compensation. I understand that these materials will be used solely for the purpose of promoting and advertising Dance Beast and its programs.

Acknowledgment of Understanding:

I have read this Waiver and Release of Liability in its entirety, fully understand its terms, and acknowledge that I am voluntarily giving up substantial rights by signing it. I acknowledge that I am signing this waiver freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
