    Looking to renew your GHC-SCW Individual and Family Plan policy? You came to the right place.

    Important Information For 2026 Plans

    • Dane County Individual and Family plan members will need to select a primary care provider in the GHC-SCW Better Together Network.

  • Read your policy carefully. You must be located in one of these counties to renew your plan:

    • Dane
    • Columbia
    • Green
    • Sauk

    How do you know where you purchased your 2025 Individual and Family plan policy?

    Check your renewal letter: 

    • If your renewal letter says "Marketplace Renewal," then select the "(FFM) Federal Marketplace (HealthCare.gov) or GHC-SCW Marketplace portal" option below.  

    • If your renewal letter says "Direct Renewal," then select the "Direct with GHC-SCW" option below.   

  • Individual and Family Plan Selection - Living in Dane County 

    Please choose one of the following plans for your 2026 Health Insurance Coverage (Please only select one plan option)

    This renewal form must be submitted before December 15, 2025.

  • Individual and Family Plan Selection - Living outside of Dane County 

    Please choose one of the following plans for your 2026 Health Insurance Coverage (Please only select one plan option)

    This renewal form must be submitted before December 15, 2025.

  • Submit Your Renewal

    If this form is returned to Group Health Cooperative of South Central Wisconsin (GHC-SCW) by November 15, 2025, your December invoice will reflect the new 2026 premium.

    If received between November 15, 2025, and December 15, 2025, your January invoice will be adjusted to reflect the new premium.

    This form must be returned to GHC-SCW before December 15, 2025, for January 1, 2026, effective date for new coverage.

    If we do not receive this form back by this date, your individual health insurance plan will renew on the plan closest to your current plan.
