  • Things to Remember

    - Reports are due on the 1st and late after the 7th of each month - If you want to save a copy of the report, click on Preview PDF before you click Submit - Your Agency # begins with an "A" and has 4 digits (A0264) after it unless you are on the Eastern Shore. If you are on the ES, your agency # begins with AES and has 3 digits after it (AES040).

  • If you are using SIMC, you only need to report numbers if you are a congregate feeding site and/or delivering to seniors. If you are using SIMC and are a TEFAP agency, you need to upload your inventory and temp log reports using this form.

  • Please enter number of meals served. If you can break it down by age group, please do. Provide number of meals served to children, adults, and seniors. If you can't break it out, list the total number in ‘Adult 18-59’. If you didn't serve anyone in an age category, enter zero in that box.

  • If your program is only congregate feeding, click submit and you are finished. Otherwise, click NEXT.

  • Numbers should match with the numbers on your USDA Monthly Report (Inventory/Intake form).

  • Pictures of your documents can no longer be uploaded. You must save the document as a pdf, Word file, Excel file.
