- Inside Wireman Apprentice
- Telecommunications Apprentice
- Advanced Placement Apprentice
*Reminder, all apprentices must be available to accept a job call immediately upon completing this form. Requested time off at the time of termination is not allowed unless you previously submitted an Off Work Notice form at least two weeks prior to your termination date.

Once placed on the out-of-work list, apprentices must remain available for a job call at all times. Failure to accept a job that has been offered to you will result in having to appear at the next scheduled board meeting.*
