Dear Parents/Guardians,

We are asking that you fill out this form if your child/children needs to be dropped off/picked up at a stop other than your home address occasionally. We ask that you fill out one form per stop, per student AM stop changes need to be submitted by 11:30 the day before the stop change will occur and PM Stop changes need to be submitted no later than 11:30 am the day the absence will take place. THE LESS CHANGES THE BETTER!
