I fully understand that these risks, known and unknown, can lead to injury, including but not limited to:

1. The possibility of discomfort and/or pain.

2. The permanence of the markings.

3.The risk of infection, scarring, and keloiding, as well as tyhe possibilty of nerve damage, and increased risk for adolescents during ertain stages of development.

4.The possibility of allengie reactions to pigments or other materials used, including jewelry, latex gloves, and/or soap.

5. The possibility of difficulties in detectine melanoma.

Having been informed of the potential risks associated with getting a tattoo. I still wish to proceed with the tattoo and freely accept and expressly assume any and all risks that may arise from the tattoo.

I confirm that I am 18 years old or older. Ink Beautiful The Lounge does not offer services to minors and will not do so in the future. To receive a tattoo or permanent makeup service, you must be at least 18 years of age.

I WAIVE AND RELEASE to the fullest extent permitted be law each the artist and the tattoo/piercing studio from all lability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors, or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from the tattoo/piercing, whether caused by negligence or fault of either that artist or Ink Beautiful The Lounge, or otherwise.Both the artists and Ink Beautiful The Lounge have given me the opportunity to ask any and all questions about tattoo/piercing procedure and the staff has answered these questions to my total satisfaction.

I affirm that both artist and Ink Beautiful The Lounge have given me instructions on care of my tattoo/piercing while its healing, and I understand and will follow them.I acknowledse that is is possible that the tattoo can become infected, particularly if I do not follow the instructions given to me.I affirm that both artist and Ink Beautiful The Lounge is not responsible for the meaning or spelling of the symbol or text that I have provided to them or chosen from the flash (design) sheets.

I affirm that I am not under the influence of alcohol or drugs and I am voluntarily getting a tattoo/piercing without duress.

I affirm that I do not have diabetes, epilepsy, hemophilia, a heart condition or take blood thinning medication. I do not have any other medical or skin condition that may interfere with the procedure or healing of the tattoo/piercing. I am not the recipient of an organ or bone marrow transplant,or if I am, I have taken the prescribed preventative regimen of antibiotics that is required by my doctor in advance of any invasive procedure such as tattoo/piercing.I agree that the Tattoo Studio has a NO REFUND policy on tattoos, piercing and/or retail sales and I will not ask for a refund for any reason whatsoever.I am not pregnant, nursing, or pumping.I acknowledge that the tattoo/piercing will result in a permanent change to my appearance and that my skin may not be restored to its pre-tattoo/piercing conditions, even after its removed.I release all rights to any photographs taken of me and the tattoo/piercing and give consent in advance to their reproduction in any and all print or electronic form. (For assurance, if you do not initial or check this provision, please inform theTattoo Studio NOT to take any pictures of you and your completed tattoo.)I agree to reimburse both artist and Ink Beautiful The Lounge for any attorneys' fees and costs incurred in any legal action I bring where either the artist or Ink Beautiful The Lounge is the prevailing party. I agree that the courts shall have personal jurisdiction and venue over me and shall have exclusive jurisdiction for the purpose of litigating any disputes arising from related to this agreement

I acknowledge that I have been given adequete opportunity to read and understand this document prior to the tattoo/piercing.

If any provision, section, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be served from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
