LIABILITY WAVIER
I herby release Ingle Ink, Artists and their agents from all manner and type of liability, claim, action, demand or compensation, in law and in equity, which my heirs or I have or might have, now or hereafter by reason of my request to be tattooed. I release Ingle Ink of all liability of any physical and mental impairment including but not limited to nerve damage, skin irritation, mental anguish, and infectious disease. I realize that misrepresentation of information provided by me is a crime and subject to prosecution. I further certify that I am an adult over the age of 18 and am not under the influence of and drug, narcotic, or substance and I am fully aware that the tattoo I am receiving is totally permanent. I make these statements entirely of free will and sound mind. I also agree to follow the procedures outlined for the proper care and healing of my tattoo. I acknowledge it is not reasonably possible for the representatives and employees of this tattoo shop to determine whether I might have an allergic reaction to the pigments or processes used in my tattoo, and I agree to accept the risk that such a reaction is possible. BRIEF DESCRIPTION OF THE TATTOO APPLICATION PROCESS
After completing the waiver form, your artist will prepare your tattoo artwork. Upon your approval, the artwork will be transferred to your skin, your artist will prepare to begin the PERMANENT BODY ART tattoo process. Using new, sterilized equipment and tools, your artist will begin the tattoo. When finished, your new tattoo will be cleaned, and wrapped in the appropriate covering. Your artist will go over aftercare techniques with you, and that will conclude the procedure.

SIGNS AND SYMPTOMS OF INFECTION
Following the aftercare instructions given to you is vitally important to the heath, and longevity of your tattoo, failing to do so can can be put you at risk for infection. Your tattoo may be infected if you notice, redness surrounding the tattoo, swelling after the second day of healing, tenderness at the site of the procedure, red streaks going from the procedure to the heart, high body temperature, and/or purulent drainage from the tattoo area. Should any of these symptoms occur, seek medical care as soon as possible.

THIS IS A LEGALLY BINDING CONTRACT
You are being tattooed in a clean environment, inspected by the California Board Of Health. All equipment, supplies and pigments are sterile and disposable. All artists are highly skilled and experienced professionals. The healing process directly affects the result and longevity of your tattoo. Please follow all the directions outlined until healing is complete. If you experience any problems with the healing of your tattoo please consult your doctor.
