US Soccer unveiled their new portal where every referee evaluation report must be submitted. That portal became available July 2025.

For those who had already completed and recieved an evaluation report before that time, please upload that PDF report below.

For everyone else, we will review the RefEval portal for your reports. Please log in a few weeks after your game to ensure your report was submitted and posted.
