Shift Swap RULES:
- Must have at least one day off within a 7 day working week
- Cannot swap shifts that will cause a nil-10 hour break period (overnight)
- If working hours in one week is over 38hrs then an approval will be requested from the SDM automatically
- Shift Swaps with less than 4 days notice will be declined
- Splitting shifts is NOT PERMITTED - the entire shift must be swapped
When completing this digital form ENSURE the following:
- Only one form needs to be submitted per shift swap.
- If one party is unable to fill the form, you can submit their name and email and the other party will be notified to fill in their part on the form.
- Please use 24H time format when entering the fields
- If you do have any working restrictions/limitations (variable hours, flexible start and finish times, reduced hours, spread of hours) specify in the comment section
