NON-DRIVER AGREEMENT

I Full Name understand SPE Group (“Company”) must complete a Motor Vehicle Record (“MVR) check before I am eligible to drive any motorized equipment owned, leased, or rented by the Company under any circumstances.

I understand that my MVR record is not acceptable to the Company or its insurers or if I do not possess a valid Commercial driver’s license I will not be permitted, under any circumstances, to operate any motorized equipment owned, leased, or rented by Company.

In view of the above restriction, I understand and fully agree I am personally responsible if I am involved in an accident, or cited for a traffic violation of any sort, while driving or operating any motorized equipment owned, leased, or rented by Company. I further authorize SPE Group to deduct from my wages any costs, expenses, fines, penalties or damages the Company incurs as a result of violating this Agreement.

I understand that in the event I am involved in an accident, or I am cited for a traffic
violation that would harm my MVR record, it is my responsibility to inform Company so that my eligibility to drive Company owned, leased or rented vehicles can be reassessed.
