WAIVER AND RELEASE OF LIABILITY
Ochoa Football Academy LLC | Norberto Ochoa Orejel

I, the parent or legal guardian of the minor participant, agree that the Participant and I will follow all rules and guidelines set by Ochoa Football Academy LLC and trainer Norberto Ochoa Orejel.

I understand that participation in soccer and related activities involves risk of injury. In consideration of my child’s participation in training sessions, clinics, or events hosted by Ochoa Football Academy LLC, I voluntarily accept all such risks and agree to release, hold harmless, and indemnify Ochoa Football Academy LLC, Norberto Ochoa Orejel, and associated staff or volunteers from any and all liability related to injury, illness, or damages.

I grant full permission for Ochoa Football Academy LLC to use photographs or video of the Participant for promotional and educational purposes across websites, social media, and other media platforms, without compensation or further approval.

I understand that this waiver is a legally binding release of liability. I agree to its terms freely and voluntarily on behalf of myself, the Participant, and our heirs and legal representatives.

If the Participant is under 18 years of age:
I agree to assume full responsibility for any medical expenses and agree to indemnify and hold harmless Ochoa Football Academy LLC and Norberto Ochoa Orejel against any claims brought by or on behalf of the minor participant.
