Lash & Brow Consent Form

By signing this agreement, I consent and agree to the perming and tinting of my natural eyelashes and/or eyebrows by my service provider.

I, initials, understand that in rare occasions, there are risks in having my natural eyelashes and/or eyebrows permed and tinted. I further understand that in rare occasions as part of the procedure that eye irritation or discomfort could occur. I agree that if I experience any of these conditions with my eyelashes, I will contact my service provider and that performed this treatment.

I, initials , understand and agree to the maintenance and after care instructions by the service provider for the use and care of my lift and tint and/or brow lamination. I realize and accept the consequences of failure to adhere to these specific instructions may cause my natural eyelashes and/or eyebrows to become brittle, or cause the treatment to not last as long.

I, initials , understand and consent to having my eyes closed and covered for the duration of the appointment for the service. I realize that time will vary depending on the thickness and length of my natural eyelashes and/or eyebrows.

I, initials , agree and consent to having picture(s) taken for promotion and marketing purposes. I understand that Elevated will never tag or mention my name or any relation of my identity to the public without my permission.

I, initials , agree to the following post care:

Lash Lifts & Tints:

• No mascara on the natural lashes for 24 hours (to allow perm to fully set)
• No oil based eyeliners or makeup on or around the natural eyelashes for 24 hours (to avoid irritation)
• Avoid eyelash curlers while having a lash lift as using them may cause your natural eyelashes to become brittle and bend unnaturally
• Use the one drip of the provided nourishing oil on the lashes every night before bed and make sure to remove all makeup from the lashes before sleeping.

Brow Laminations:

• Avoid contact with water AND brow products on the brow area for 24 hours after the appointment to ensure the lamination has fully set.
• After the first 24 hours, keep your brows moisturized with the provided oil to keep the hairs from becoming brittle or bending unnatrually.
• Be aware that when you sleep on your brows, they will probably look slightly untamed when you wake up. Use a clear brow (soft or strong hold) gel to brush them back into place.
• When getting out of the shower, (after washing them) style brows how you want them to look as soon as possible, as they will most likely appear how they dry.

This agreement will remain in effect for this service and all future follow-ups conducted by Refine Skin Spa.

I understand that this consent agreement is legally binding. I have read and fully understand all the information of this agreement. I am over the age of 18 and consent to all of the above.

I have read and filly understand Refine Skin Spas policies via the website and agree to adhere to them.
