Payment Terms*

Vendor understands and agrees that payment terms for all DeSantis vendors are net 30 days, and while DeSantis generally sends payments for vendors weekly, payment cannot be guaranteed faster than 30 days.

Insurance Requirements*

Vendor has BOTH general liability insurance of at least $1 million and workers' comp insurance of at least $500k. Being a sole proprietor with a state exemption does NOT waive this requirement for DeSantis vendors.

The insurance certificates must be uploaded for DPM records. If you have both general liability insurance and workers' comp insurance on a single insurance certificate, then attach the same certificate in both fields below.

WARNING: Submitting insurance certificates that are missing the required insurance will result in immediate denial. DPM will not follow up to collect documents, so be sure to submit the correct documents when submitting this form.
