    Athlete Program - Application Form [Juniors]

  • JOIN THE PROGRAM THAT SUPPORTS SOME OF THE BEST JUNIOR ATHLETES IN THE COUNTRY.

    The Athlete Program has 3 key aims…

    1. To make athletes strong, robust, and resilient.
    2. Improve key metrics for strength, power, speed, and change of direction.
    3. To develop better movement strategies, and more options in their sport.

    We do this by combining experience, data-driven processes, and on-field performance within individual programming.

    We first identify the physical qualities we need to prioritise, we then take those new qualities into skill development, and finally help athletes develop the will to use these for better performance in the sporting environment.

    What's included;

    • 2 x 45 minute training sessions per week
    • Force plate testing for the highest level of accurate data
    • Conditioning/energy system training
    • Ongoing testing metrics and reports to track progress 
    • Sprint/acceleration technique and development
    • Plyometric, speed and power training
    • Teaching how to use greater athleticism in their sport
    • Nutrition overview and advice
    • Team building and mindset activities
    • Plus - DISCOUNTED rehabilitation or injury management treatment sessions*

    REQUIREMENTS

    1. Athlete Age: 11-18 years
    2. Location: Mackay, Queensland.
    3. Weekly session commitment: 2 sessions/week
    4. This program is not designed for absolute beginner or novice juniors; we require a minimum of 12-18 months competing in their sport.

  • SPORTING DETAILS and HISTORY

    This gives us a complete picture of your child's exercise and sporting background, and any other relevant information.

  • SKILLS / SPORT COACH DETAILS

    From time to time, Gavin may require discussions with your child's skills or sport team coach in order to provide tailored, adjusted, or specific seasonal adjustments to programming.
  • INJURY / HEALTH HISTORY

    Injuries and health concerns can affect what we can build into training and athletic development. Please provide as much information as you can.
  • TRAINING WINDOW AVAILABILITY

    Note that training times are NOT unlimited, please check your child's activity schedule before selecting two (2) requested time slots to join. Training sessions are 45 minutes duration. 
  • PAYMENT AND TERMS

    You will be contacted directly for payment to secure your place. Fees for junior athletes in the program are $60/week.
  • TERMS AND CONDITIONS

    • Once you/your child has been accepted on the Health & Performance Athlete roster, payment indicates a commitment to the included sessions and adherence to both training standards and times.
    • A minimum of 2 appointments are optimal and preferred, to achieve your goals. Should you desire extra times please discuss with Gavin as soon as possible as this is an optional extra and will incur an additional fee.
    • Respecting coach time - be ready before your session, if you are early, please wait patiently for your start time.
    • Upfront payment of four weeks in advance is required to secure a place on the program. This payment is non-refundable.
    • Missed session policy: Please notify Gavin Heward directly if your child cannot make it to their training session. Missed training sessions with less than 48 hours notice will be forfeited. If you know prior to 48 hours that your child will be absent, let us know and we will arrange makeup sessions.
    • In the event your child is injured, please notify us immediately so we can arrange training/rehab appropriate to the situation.
    • Gavin Heward is contactable during standard business operating hours only, and will respond to messages within 24 hours.
    • By participating in this program you give consent for images/videos containing your child to be used in promotional materials including but not limited to flyers, advertisements, and on the Health and Performance by Gavin Heward website and social media channels. No specific identifying details will ever be published about any individual working with Health and Performance by Gavin Heward without prior consent.
