Horse ID Number Application

Arizona Versatility Ranch Horse Association
Each horse ridden in approved AzVRHA events must be assigned a permanent AzVRHA Identification Number. This number will be lifetime identification and remain with the horse regardless of ownership.

Prior to participation in an approved AzVRHA event, a completed Horse Identification Number Application Form must be provided to AzVRHA (either to the office prior to the event or to show management at the event The Ranch Horse Identification Number will be assigned to the horse by AzVRHA. The rider will be responsible for providing the Horse ID Number at all following approved events.

Ownership as applied for, must correspond with the ownership on the registration papers (photocopy of transfer documents must accompany photocopy of breed registration papers showing that a transfer is in progress).

- Non Breed Registered Horses must provide (3) 4x6 photos, one each of full left side, full right side and a head view that includes all identifying markings and brands.
- Junior Horses (5 years old or younger) must provide a Veterinarian Certificate of age as follows: (or - A copy of a Coggins laboratory test result properly executed to include age may be substituted)
