Please submit this form at least TWO WEEKS before vacation.

Each family is eligible for one 50% off week after six months of enrollment. Each 6 months of enrollment you earn another 50% off week. Two half-price weeks may be combined if eligible. Discounted week may not accrue each year. Vacation weeks are for families who will not be in attendance at Stepping Stones for a consecutive week Monday-Friday.
