    Welcome back! I'm thrilled for you to join us as we dive into a wonderful year of artistic expression and creativity!

    To ensure a smooth start, please prioritize the included paperwork. It's crucial for understanding each student's needs and coordinating our activities. Timely return of these forms will help us swiftly shift our focus to our shared passion: making beautiful music together.

    This orientation packet includes the following important documents and their respective uses. Use this as a checklist for ensuring that you've completed all necessary paperwork.

    • Student Information Form: Information collected to establish clear lines of communication to support student activities over the course of the year.
    • SCPS Medical & Travel Form (requires Notarization): Fulfills the SCPS requirements to have pertinent medical information in the possession of the teacher and designated adults during band events, especially when traveling.
    • Band Student Uniform Agreement Form: Establishes the understanding that students are responsible for the reasonable care and maintenance of the school-owned band uniforms, which are quite expensive.
    • Band Assessment Form : Communicates the operating costs of the band in order to identify the necessary costs to be assessed to each band student and their families. (This does not need to be turned in)
    • School Instrument Rental Form: Includes the information needed to release a school-owned instrument into the care of a student. This is only required of students that will be renting an instrument from the school (this includes all percussionists)
    • Assessment & Accessory Order Form: Lists the required and optional items and accessories that students will need in order to perform and participate in the various band activities. It provides spaces to indicate the sizes for uniform accessories and allows for the ordering of extra accessories if desired. Finally, it allows families to choose the various ways in which these assessments may be paid.

    This document will be taken on all trips and band functions.

    It is the responsibility of the parent to see this is properly executed (notarized) and returned to Mr. Langston by July 29, 2024.

    I/We the undersigned, being the parent, legal next-of-kin, or legal guardian of:

    • I/We also authorize emergency medical treatment for this student beginning June 1, 2024 and continuing through May 31, 2025.
    • I/We acknowledge the liability for medical expenses, hospital expenses, and/or other charges incurred for such services as may be rendered for or on behalf of my/our child as a result of injury or sickness.
    • I/We will assume financial responsibility for the incurred expenses either personally or through the insurance company listed below.
    • I/We understand that if my/our child is injured or becomes sick, Seminole County Public Schools Florida will not be liable unless the injury or illness is the result of negligent conduct on the part of an employee of Seminole County Public Schools, Florida.

  • LMHS Band Student Uniform Agreement Form, 2024-2025 For Instrumentalists Only

    Band uniforms are quite expensive (over $400.00 when purchased individually) and thus must serve us for an extended period of time. We are currently in the fourth year of the life of these uniforms.

    To extend the lifetime of the uniforms as much as possible, each student must assist in caring for them. Uniforms are kept at school and distributed before each performance. Students are given instructions on proper wearing and care of each part of their uniform.

    The Band Assessment covers regular uniform cleaning and minor repairs. Students are responsible for damage caused by carelessness or improper wearing/storage of the uniforms. Hat boxes are issued with the uniform hats, and travel bags are issued to carry other accessories. Students are given clear instructions on what should and should not be kept in their hat boxes and band bags. Paying attention to instructions, meeting deadlines, and taking care of obligations is a fundamental skill and responsibility. This is an important part of education and life, not just the band experience.

    Failure to follow any instructions pertaining to the uniform and/or failure to return the hat box and hat, clean, when requested, will be handled as a failure to perform proper maintenance. This can result in the student being charged for any repairs or replacements needed as a result of improper maintenance, underscoring the importance of compliance. This charge can be included as a part of a student's debt, appearing on the senior debt list.

  • LMHS Band Assessment Form, 2024-2025

    Band Assessment monies cover band operating costs such as registrations, clinicians, music, travel and many other expenses required for the band to function successfully. To provide some perspective, The annual cost of operations for the LMHS Band averages around $100,000.

    To meet these expenses, we do a combination fundraising activities:

    • Collecting a "Fair Share" Band Assessment from each student
    • Collecting Instrument Rental Fee from students using school-owned instruments
    • Selling concessions at football games
    • Conducting several other fundraisers throughout the year

    Fair Share Assessments and instrument rental fees cover about 35% of the band's operating expenses. The remaining $65-70,000 must be raised via fundraisers.

    Monies raised from these sources are used to provide the following materials and/or services to our students:

    • Music Performance Assessment fees
    • Leadership, Rookie, and Band Camp expenses
    • Travel expenses
    • Instrument-specific clinicians
    • Instrument purchase and repair
    • Uniform dry-cleaning

    New student financial obligations for the first year are slightly higher because they also need materials for marching band rehearsals and performances that returning students already have, including:

    • Rehearsal shorts
    • Performance gloves
    • Performance socks
    • Poncho
    • Travel bag

    Based on the rising cost of materials, the assessments for the 2024-2025 school year are as follows:

    • Band Assessment for all students: $325
    • New student accessories: $75 (for marching band accessories & uniform components)
    • School Instrument Rental: $50 per semester ($100 for the year)

     

    Below are the various options for how families can pay the Band Assessment.

    Note: A payment plan cannot be offered for the purchase of items and accessories. Because we have to place orders with vendors by a deadline, we cannot accept payments for these at a later date.

    • Option 1: Pay the assessment in full.
      • Pay at Orientation Night with card, cash, check, or money order
      • After orientation, pay online via payment link OR send in payment to Mr. Langston
    • Option 2: Pay an initial deposit and the balance at a later date.
      • Deposit of $200 due by Wednesday, July 24, 2024
      • Deadline for balance to be paid by Friday, August 23, 2024
    • Option 3: Request information on financial assistance.
      • The Booster Treasurer will contact you to set up a plan for payment, which could include fundraising, scholarship, sponsorship, or whatever means are currently available.

    REMEMBER: It is our mission that a family's financial status should NEVER be a reason for a student to NOT take part in band! Please contact us if you need assistance!

    The choice of payment option can be made on the Assessment & Accessory Order Forms.

  • SCPS Instrument Rental Form, 2024-2025

    The following form must be filled out by any student that will be using a county/school-owned instrument for the school year. There is also a $100 Instrument Rental Fee that is used to repair and maintain these instruments.

    The primary instruments that can be rented from the school are: Oboe, bassoon, French horn, baritone, tuba, & percussion

    The following instruments may be rented from the school if available: Flute, clarinet, alto sax, tenor sax, trumpet, trombone

    The following instruments may be rented or assigned to meet instrumentation requirements: Bass clarinet, baritone sax, flugelhorn, English horn

    All percussion students are responsible for this form and the fee for the use of the school's percussion instruments.

  • APPROVAL OF OFF CAMPUS USE OF SCHOOL BOARD PROPERTY

  • STUDENT/PARENT/GUARDIAN STATEMENT OF RESPONSIBILITIES:

  • 1. I will take reasonable precautions to maintain the security of the equipment in my home.

    2. If the equipment or any of its accessories are lost or stolen, I will immediately notify the police and the school and department mentioned above.I agree to pay the inventory price if the equipment is not recovered or damaged beyond repair.

    3. The equipment and accessories will be returned no later than the last day of student attendance.

    4. If I experience any problems with the equipment, I will notify the school and department immediately and explain what is wrong with it. If the damage occurred while in my possession, I agree to pay for the repair.

    5. I will see that the equipment is used appropriately for the educational purpose intended.

    If you are completing this at orientation, a pdf of this form will automatically be sent to Mr. Langston's email address. To get a printed copy, proceed to the Printing Station to have the band booster print your form.
