2024-25 Universal field trip permission

Throughout the school year our students may have the opportunity to leave school for an educational experience related to their learning.

Your child(ren)'s teachers may notify you in advance of each field trip with details and necessary information.

By signing this form, the parent or guardian gives permission for their child to participate in these school-sponsored events.

I hereby give permission for my child to participate in field trips sponsored by the Jewish Community Day School of Rhode Island. I understand that these trips will be appropriately chaperoned by JCDSRI faculty and staff.
