Rams Registration Form

Welcome to the Mount Douglas Rams Football Program. This Registration Form contains necessary information required to complete student/athlete registration to play football for the upcoming season.

Please take the time to read all sections of the form and complete them in full (regardless of if the student/athlete played last year). Incomplete registration forms will not be accepted.

First Time Players: Please make sure you include a photocopy of your birth certificate, care card, driver’s license or passport.

Returning Players: Returning players will have theirs on file from previous seasons. All new students wishing to play football at must meet Mount Douglas Secondary School eligibility criteria in order to be registered with BC School Sports.

Please note:
•       The registration fee is $940.50.
•       ALL Documentation and Fees are required at the time of registration.
•       ALL cheques are made payable to Mount Douglas Secondary School.

PLEASE ENSURE YOU HAVE COMPLETED & SUBMITTED THE FOLLOWING WITH YOUR REGISTRATION:

1) Completed/Signed Registration Form (required every season)

2) Proof of Age (FOR NEW PLAYERS ONLY) - Please provide a copy of one of the following for the Player: Birth Certificate, Care Card, Driver’s License or Passport

3) Completed/Signed Medical History Questionnaire (required new every season)

4) Signed Concussion Awareness (required new every season)

5) Signed Code of Conduct (required new every season)

The following DEPOSIT CHEQUES are ALSO required at time of registration.

(Deposit cheques are destroyed when gear and jerseys are returned):

1) 400.00 Gear Deposit Cheque (Post Dated for 31-December-2025)

2) $200.00 Jersey Deposit Cheque (Post Dated for 31-December-2025)

3) $200.00 Volunteer Deposit Cheque (Post Dated for 31-December-2025)

REGISTRATION FEE:

The registration fee to play with RAMS = $940.50.

There are 4 options for registration fee payment:

OPTION #1: Pay in FULL via School Cash Online

OPTION #2: Pay in INSTALLMENTS via School Cash Online ($313.50 due in September, October & November)

OPTION #3: Pay with POST DATED CHEQUES ($313.50 due in September, October & November)

OPTION #4: KidSport (KidSport covers up to $700, Pay $240.50 Balance owing via School Cash Online)
