Rams Registration Form

Welcome to the Mount Douglas Rams Football Program. This Registration Form contains necessary information required to complete student/athlete registration to play football for the upcoming season.

Please take the time to read all sections of the form and complete them in full (regardless of if the student/athlete played last year). Incomplete registration forms can not be accepted.

New Players: Please make sure you include a photocopy of your birth certificate, care card, driver’s license or passport. All new students wishing to play football at must meet Mount Douglas Secondary School eligibility criteria in order to be registered with BC High School Football and BC High School Sports.

Returning Players: Returning players will have their ID on file from previous seasons.

Please note:
•       The registration fee is $940.50.
•       ALL Documentation and Fees are required at the time of registration.
•       ALL cheques are made payable to Mount Douglas Secondary School.

PLEASE ENSURE YOU HAVE COMPLETED & SUBMITTED THE FOLLOWING WITH YOUR REGISTRATION:

1) Completed/Signed Registration Form (required every season)

2) Completed/Signed Medical History Questionnaire (required new every season)

3) Signed Concussion Awareness (required new every season)

4) Signed Code of Conduct (required new every season)

5) Proof of Age (FOR NEW PLAYERS ONLY) - Please provide a copy of one of the following for the Player: Birth Certificate, Care Card, Driver’s License or Passport. Can be emailed to mdramscoach@gmail.com.

The following DEPOSIT CHEQUE is ALSO required.

1) $200.00 UNIFORM Deposit Cheque

Post Dated for 31-December-2026
To be collected prior to uniforms being signed out
Deposit cheques are destroyed when gear and jerseys are returned

REGISTRATION FEE:

The registration fee to play with RAMS = $940.50.

There are 3 options for registration fee payment:

OPTION #1: Pay in FULL via School Cash Online, Cheque or Debit/Credit (In Person School Accounts Clerk office) in September.

OPTION #2: Pay in INSTALLMENTS via School Cash Online, Post Dated Cheques or Debit/Credit ($313.50 due in September, October & November)

OPTION #3: Financial Assistance (e.g. KidSport). For example, KidSport covers up to $700 per year. Pay Balance Owing $240.50 via School Cash Online, Cheque or Debit/Credit
