F45 Tauranga Membership Suspension Form
Here are a couple of things you need to know about membership pauses:
- We do not do indefinite pauses, we need an end date. (Exemption for those pausing due to sickness, pregnancy or injury.
- There is no retrospecitve pausing (ie suspensions cannot be backdated)
- We require 1 weeks notice
- Minimum pause period is 2 weeks
- Each suspension will incur a one-off $15 suspension fee (exemption for sickness, injury or pregnancy)
- In any 12-month period you may freeze your membership for up to 3 months

Please note if you select a date just after your weekly payment is scheduled, your returning payment will be delayed.
Pause END date. The date you want your pause to end ie the day before you return to the gym (DD/MM/YYYY)*

$15 suspension fee will be charged to your exisitng membership account
