This form is not for observed meets.
This form is for requesting an Approval for a Spring/Summer 2026 Meet.
Only one application per form allowed.

If you have to submit more than one meet to be approved, you will be redirected to add another application when complete.
- You will need the USAS Member ID's for the Meet Director, Meet Referee and Admin Official to complete this form. Credentials will be verified prior to issuing a sanction/approval. If the member's credentials are not valid, a sanction/approval may not be issued until brought into good standing. Any planned/named assistants in the meet information will be verified independent of this application.
- Please provide contact information for the person submitting this application. THE PERSON SUBMITTING THE APPLICATION WILL BE THE PERSON WSI COMMUNICATES WITH
- Please DO NOT load your meet event files with the application. Instructions will be given on how/where to submit the event file AFTER the sanction has been issued.
- Be sure to review the Guide to Meet Packet Language to ensure all the required information is in the meet info.
- Reference the Meet Operations Info sheet for any questions regarding assistants in position
- Requests for Approvals MUST be submitted 30 days or more before the meet start date.
- A 2025 FORM D must accompany the Approval Request. USAS changed the form in December 2025; all Form D's submitted with the meet bid are invalid and a new form must be completed for approval by USAS.
- Please allow up to 7 business days for an Approval to be processed and finalized.

I have reviewed and will follow all deadlines published and my meet information complies with all currently posted policies.In applying for this approved event, the Host Club agrees to comply with the conditions of approval as mandated in the USA Swimming rule book & Wisconsin Swimming policy manual.All participating athletes and clubs do not need to be registered with USA Swimming. If any participants are competing from another LSC, I, as the meet host, have formally received permission from the LSC that registered the other athletes and/or clubs and will provide documentation of permission requests prior to meet recon being done, if needed.Wisconsin Swimming retains the right, for any changes in health and safety considerations, or in compliance with governmental or health department guidance or rules, to revoke this approval at any time on the authority of the General Chair or the Vice-Chair for Program OperationsI certify that information contained on this application is correct and the proposed facility meets all the requirements of Article 103 of USA Swimming Rules and Regulations for the requested season.
