  • HAIR LOSS SERVICES — CLIENT AGREEMENT & AFTERCARE

    1. Consultation and suitability. This information applies to Mesh Integration, Meshless Integration, partial systems and custom scalp prostheses. Completing this form does not confirm suitability, book installation or authorise a custom order. The appropriate solution, care requirements and costs will be discussed during consultation. These cosmetic solutions do not diagnose or treat the cause of hair loss or guarantee natural hair regrowth.

    2. Health information. I agree to disclose relevant medical conditions, scalp concerns, allergies, medications and treatments and report changes before appointments. Services may need to be postponed or adapted, or medical advice requested, if suitability is uncertain. Hair integration and scalp prosthesis services are not available during active chemotherapy or radiotherapy.

    3. Aftercare. I agree to follow professional instructions for my chosen method, including washing, drying, brushing, sleeping, exercise, swimming, suitable products and heat styling. I will discuss colouring, chemical treatments, alterations and removal with Catherine beforehand. Home removal or adjustment should only be undertaken where specifically instructed.

    4. Maintenance. Regular appointments are necessary. Frequency depends on method, attachments, natural hair growth, scalp health and wear. My schedule and expected costs will be explained before installation. Delayed maintenance can increase tangling, matting, lifting, discomfort and damage.

    5. Shedding and comfort. Natural hair continues to shed and some shed hair remains within attachments until maintenance or removal. Tension, unsuitable attachments, matting and irritation may contribute to breakage or additional hair loss. I will promptly report pain, persistent itching, redness, sores, unusual shedding or other changes.

    6. Longevity and wear. Lifespan varies with construction, materials, hair colour, attachment, lifestyle and aftercare. Shedding, tangling, colour or texture changes, lifting and base wear may occur over time, and repairs, adjustments or replacement may be needed. Expected wear will be discussed. This does not remove rights where goods or services fail Australian Consumer Law guarantees.

    7. Products and exposure. I will follow product and heat guidance. Colouring, excessive heat, chlorine, seawater, sunscreen, tanning products and environmental exposure may affect hair, base or attachments. Method-specific precautions will be explained.

    8. Costs. Initial service and relevant maintenance, repair and replacement costs will be explained before proceeding. Additional paid work requires agreement before it is carried out. Remedies owed under Australian Consumer Law are not ordinary paid maintenance.

    9. Custom orders. A custom system is only ordered after separate approval of specifications, price, deposit and payment terms, estimated timing, and amendment and cancellation conditions. This form alone does not authorise an order. Changes after approval may affect cost or timing; additional charges require agreement.

    10. Deposits and cancellations. Any required deposit, notice period and cancellation charges will be agreed before booking or ordering. Change-of-mind alterations or cancellation may be limited once custom production starts. Any retained amount or charge must be lawful, reasonable and reflect work completed, unrecoverable costs and losses reasonably avoided. Deposits are not automatically forfeited in every case. Statutory cancellation and remedy rights are preserved.

    11. Consumer rights. Nothing excludes or limits Australian Consumer Law rights, including applicable repair, replacement, refund, cancellation or compensation rights. I will contact Catherine promptly about concerns for assessment.
