Permission Slip for Field Trip
Washtenaw Alliance for Virtual Education offers an alternative program to its students. Part of our program encourages students and staff to use the community as a classroom and opportunity for learning. Opportunities to visit exhibits, performances, college programs, and take P.E. walks often arise on short notice.
This permission slip will allow your student to attend all "close to home" events as they come
Students may be responsible for their own transportation (via city bus or private vehicle) WAVE may provide bus transportation for certain events. Parents/guardians will be notified if there is bus transportation.
Ihereby give my son/daughter/ward permission to participate in local field trips during the current school year.
I understand students are responsible for their own transportation for many trips.
Irelease and hold harmless WAVE and its employees from any and all liability in connection with their transportation and attendance at any field trips.
I assume full responsibility for any damage to person or property caused by my student. I agree that if it is determined that my student needs medical or dental treatment, I will be responsible for any such treatment determined necessary by a physician or dentist. I further agree that if the behavior or health of my student should make it necessary to send him/her/they home prior to the end of the field trip, I will be responsible for those expenses. I understand that no student will be sent home without notification to the parent/guardian.
