Aknowledgement of Risks: I, the undersigned, understand that participating in basketball involves inherent risks, including but not limited to physical injury. I acknowlege that I am voluntarily participating in this activity with knowledge of these risks.
Release of Liability: In consideration of being allowed to participate in Dub C Fest: Alumni Hoopfest, I hearby release and hold hard harmless West Charlotte High School and West Charlotte High School Class of 2004, its officers, employees, and affiliates from any and all claims, demands, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in this activity.
Medical Consent: I authorize West Charlotte High School Class of 2004 to seek medical treatment on my behalf in case of an emergancy. I confirm that I have adequate heath insurance coverage.
Code of Conduct: I agree to adhere to the code of conduct as outlined by Charlotte-Mecklenburg Schools and understand that failure to comply may result in my removal from the activity.
Photography/Video Consent: I consent to the use of photographs or videos taken during the event for promotion purposes by West Charlotte High School and West Charlotte High School Class of 2004.
