Payment Plan Descriptions

Four (4) Installments: 25% of the amount due (Only for 12 or 16 week courses) + 3% credit card fee
- The 3% processing fee and $35.00 payment plan fee are divided equally across four installments.
- The first installment is processed immediately or within 3–5 business days and includes ¼ of the $35.00 fee.
- Remaining installments are processed monthly on the same calendar day.
- If a month does not have the same calendar day (e.g., January to February), payment will process on the last day of that month.

Two (2) Installments: 50% of amount due + 3% credit card fee
- The 3% processing fee and $35.00 payment plan fee are divided equally across two installments.
- The first installment is processed immediately or within 3–5 business days and includes ½ of the $35.00 fee.
- The second installment is processed one month after the first installment on the same calendar day.
- If a month does not contain the same calendar day (e.g., January to February), the payment will be processed on the last day of that month.

If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day.

By signing this form, you agree to the terms and conditions outlined herein and understand the following:

1. Non-Refundable Charges: Tuition payments made through this authorization are non-refundable unless specified by the United States Sports University, Inc. refund policy. It is your responsibility to be aware of and understand this policy.

2. Authorization Validity: This authorization remains in effect until the total amount due has been paid in full or until the approval is revoked by the cardholder in writing. Revocation must be provided to United States Sports University, Inc. at least 15 days before the next scheduled payment.

3. Payment Delays or Failures: United States Sports University, Inc. is not responsible for any delays or failures in processing payments due to incorrect or outdated information provided by the cardholder, or due to issues with the cardholder's financial institution. Any resulting late fees or penalties are the responsibility of the cardholder.

4. Payment Plan Selection: The cardholder is responsible for selecting the appropriate payment plan (4 installments or 2 installments) and ensuring that sufficient funds are available on the specified due dates. Failure to meet the payment deadlines may result in additional fees or penalties as per United States Sports University, Inc. policies.

5. Privacy and Security: United States Sports University, Inc. will take all reasonable precautions to protect the cardholder's information. However, the cardholder acknowledges that no method of transmission over the Internet or electronic storage is 100% secure and agrees to hold United States Sports University, Inc. harmless in the event of any data breach or unauthorized access.

Changes to Terms: United States Sports University, Inc. reserves the right to modify these terms and conditions at any time. Any changes will be communicated to the cardholder in writing. Continued use of the payment plan constitutes acceptance of the revised terms.
