This fee also allows us to hold the puppy for you and mark them as reserved in your name until the adoption process is complete. Agreement and Acknowledgment
I understand that completing this application does not guarantee I will be allowed to adopt a puppy, and that the application is a tool to help Donna's Cavapoo Family determine if this puppy and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I understand that any misrepresentations may result in my application not being approved and/or removal of the adopted puppy from my possession. I also certify that I have never been convicted of animal cruelty or neglect.

By submitting this application, I agree to the terms and conditions of the adoption process and acknowledge the refundable reservation fee. I understand the needs and characteristics of the Cavapoo breed and commit to providing a loving and responsible home. If, for any reason, I can no longer care for the puppy, I agree to return the puppy to Donna, the breeder.
