Family History Form

The Sesquicentennial Committee is collecting family histories for those who have made Anita their home at any time, and especially in the last 25 years, to update our town history to include in the 2025 History Book.

Submit your family history in one of two ways:

1. Fill out the information form below. Include as much or as little information as you want to provide. Upload photos near the bottom of this form.

2. Fill in your contact name, email address, first and last name. Submit your written Word document at the bottom of this form.

Family Histories are due by October 1, 2024.

Family Photos

You may submit 3 photos of your family. All photos may not be used due to picture quality or space availability in the book. We will do our best to include all photos.

For all photos/images:Please ensure that you have permission from anyone that is shown in a photo.

Please name the photo file with the name of your family included. (example: joesmithfamily1.jpg and joesmithfamily2.jpg)
If submitting photos from a cell phone, please submit photos in the highest resolution possible (original size).
All images should be in JPG format.
To maintain image quality of physical photographs, photos/images can be scanned at the Anita Public Library and then uploaded here.
