Once you finish this questionnaire, I will be reaching out via email if I am able to accept your custom tattoo request. I will be booking appointments Tuesday-Saturday, in the morning and afternoon. Please specify what days/times you are available during that timeframe. Please note, Fridays and Saturdays are the most frequently requested days and they go quickly, so I cannot guarantee an appointment on those days. If you request a Friday or Saturday, please list another day as backup. I will always suggest the soonest date that I have open within your preferred availability.* This field is required.

If I am able to accept your tattoo request, I will send you an appointment date and time based on your availability listed above. I will request a $50 deposit to reserve your appointment. Deposits are non-refundable but they will go towards the final cost of your tattoo.* This field is required.
