Registration

Registration begins the moment this jot form goes live, and ends August 13th, 2026. This service is for caregivers with custody of chlidren. The program services Port Colborne and Wainfleet residents. Please only submit one request per household.

Process

Once you fill out this application, our staff will connect with you via email to confirm approval. Pick ups will start on the day of our back-to-school event (August 20th) and the last day for pick up will be August 31st, 2026. 

**Supplies and backpacks will be provided on a first-come first-serve basis.

 

For the next step, please fill out the information for each child applying for the back to school program.

Please know that the families proof on income, proof of residency and identifications for themselves and children are required in order to submit the application for the back-to-school program. This step in the process will be required on the last page. If unable to provide it at this time, please "Save" this application for when you do have the required documents. • Proof of residency
• Identification for Caregivers and Children
