1. Please complete this form, attach proof of vaccination and submit.

2. Once we have received your registration and your vaccination certificate, we will send you an email confirming class date and time.

We will also contact you if you are missing anything, or if we have any questions about your application. Please note: Applications can take 1-2 business days to process. We do our best to get back to you ASAP.

Please make sure to include up-to-date vaccination records.

Each client must pre-pay class fees in order to reserve a spot. Please note that fees are non-refundable. (In the event of low registration and a subsquent change of a class start-date, clients may request a fee reimbursment).

Direct Contact Dog Play Waiver and Agreement

I have been informed of the risks of possible injury or contagion including canine flu associated with direct contact with other dogs. I also understand the importance of dog-dog play for my dog. I have weighed those two things and decided to allow my dog to have direct contact with other dogs during Brooklyn Pup LLC dog training classes. I hereby accept and assume any and all risks that would customarily and ordinarily occur by my dog’s participation in play or as a passerby before, during, and after our classes. I hereby agree to indemnify and hold harmless Brooklyn Pup LLC, Sari Carel, and all of their employees, contractors, volunteers, clients, and associates of any and all claims of injury, expense, costs, or damages due to our participation in classes.
