In consideration of my participation in the Choose to See: 5K Walk/Run, I, the undersigned participant, hereby acknowledge and agree to the following:

I release, discharge, and hold harmless the organizers of the Choose to See: 5K Walk/Run, their directors, employees, volunteers, sponsors, and any other persons or organizations associated with the Choose to See: 5K Walk/Run from any and all claims, damages, or injuries, including death, that may arise from my participation in the Choose to See: 5K Walk/Run or as a result of my use of any facilities or equipment provided by the Choose to See: 5K Walk/Run.

CADAll 5K registrants are expected to pay a $50 Registration Fee. If unable to pay online, the fee will be $60 to register on the day of the event. All 2K registrants are expected to pay a $25 Registration Fee. If unable to pay online, the fee will be $35 to register on the day of the event.
