    Please get in touch with your child's audiologist and the two people writing recommendation letters. Upload the documents here as separate PDFs. Important: You cannot submit the application until the audiogram and recommendation letters are uploaded.
  • Audiogram

    • Your child must have bilateral (two-sided) moderately severe or greater hearing loss to be eligible. 
    • If your child uses hearing aids, we need an unaided audiogram performed within the last 12 months. 
    • If your child uses cochlear implants, we need the most recent programming report or audiogram.

  • Letter of Recommendation from a Professional

    You will need one letter of recommendation from a professional who knows your child. This could be a therapist, teacher, or hearing health professional, like a speech-language therapist or a listening and spoken language specialist (LSLS).

    You will need one letter of recommendation from a teacher or a non-family member who knows the child well and can describe the benefits of the selected program for your child.

  • AG Bell may want to contact your family to learn about your child's progress. We may also want to write a story about your child or your family. Sometimes, we may ask for your help in supporting school and health policies where you live.

    Note: If AG Bell contacts you about being part of a media story, you can choose not to participate.

  • Payment Method Notice

    This year, all financial aid awards will be distributed via ACH (direct deposit) rather than paper checks. If awarded, you will be required to set up a secure vendor account through BILL.com, where you will enter your banking information. This process provides a faster and more reliable payment experience than mailed checks.

    Please note that the parent or guardian receiving the funds must remain the primary point of contact each year you apply, as your banking information will be linked to your individual vendor profile. 

  • NOTICE

    If you live in the United States or its territories and are chosen to receive an award, the money may be taxed. The award could impact your ability to qualify for Supplemental Security Income (SSI). To find out if you need to pay taxes, we suggest talking to your accountant, tax lawyer, or tax preparer.

    If you live in the US or its territories and are selected for an award, the government requires us to collect a tax ID or social security number before we can send the check. 

    We need this information for all awards of $600 or more, as required by the government. AG Bell will keep this information private and will only use it to file a tax form. The review committee will not see this page of your application.

    If you live in the United States and cannot provide a social security or tax ID number, you will not be able to receive an award. This is a rule from the United States government, and we cannot make any exceptions.
