Termination Form
This form is to be used if you are Terminating an Employee with cause only. This form must be downloaded as a PDF AND uploaded into Workday when you are initiating the termination business process. Any termination with cause submitted will be denied without the upload of this document. The HR team should be made aware of terminations with cause BEFORE submitting/initiating the termination.
Definition of Terminating with Cause
Termination with Cause is when you are terminating an employee from the company for a reason that is Involuntary. This does NOT include No Call, No Shows as company policy states if an employee is a No Call, No Show for 3 consecutive days, that is considered a voluntary termination.
