Dear Parents:

On behalf of Fairmont Independent Schools, Global Bridge Associates will be visiting the homes of our international students at least once per semester to check on student living conditions and to verify that students are living with one or both of their parents.

Verification will be determined through the collection of the following documents and successful completion of home visits conducted by Global Bridge Associates:

1. A picture with your child inside the residence (picture may be with one parent or both)

2. A copy of your Driver's License or Passport with photo ID

3. A copy of two utility bills with your name and address visible on the bills (water, gas, electric)

4. A signature on this verification form stating you will not allow your child to live alone.

I agree to notify Fairmont Schools within forty eight hours if there is any change in the status of our residency.

I understand that Global Bridge Associates on behalf of Fairmont Schools will conduct visits to our residence at least once a semester.
