• Catalog change request form & User Account Request form

  • During the role out of the NMT electronic catalog, this form will be used for request changes or to inform of typos. 

    This form will serve as a user account set-up form if the requestor is an approved role to request changes.

    User set-up to request and submit changes is a one-time process to create a login account for the NMT catalog where future updates for the catalog can be requested.

    If you are only informing of catalog typos, no account is needed, and our office will correct typos being submitted. (this type of request can be completed at any time)

     

    This form is currently open and accepting submissions for processing. Processing will be completed as follows for Preliminary Catalog postings: 

    • Submissions from the periods of September to deadline of July 14th will be entered into the current upcoming academic catalog.

     

    • Submissions after the deadline will be held and processed for the following years academic catalog

     

    Projected live posting of Catalog is August 4th.

  • The NMT Registrar Office will receive your request form and process the set-up of a catalog account if an in approved role to request updates to the catalog (approved roles examples are: Directors, Deans, Chairs, designees, etc).

    Once a login account for the catalog is created, you will receive login information along with instructions for navigation iand processes of the program.
